What Happens If You Don't Replace a Missing Tooth?

A missing tooth, especially one at the back of the mouth, is easy to ignore. It does not always hurt, it may not show when you smile, and daily life goes on as usual. But dental research shows that an empty tooth space rarely stays "neutral." Over months and years, it can quietly reshape the bone, the bite, and the surrounding teeth. This article looks at what the evidence actually says, and when replacement matters most.

What happens to the jawbone?
The root of a tooth transmits chewing forces into the jawbone, and this stimulation helps maintain bone density in that area. Once a tooth is extracted, that stimulation stops, and the bone begins to remodel. A classic 12 month clinical and radiographic study by Schropp and colleagues found that the width of the residual alveolar ridge can be reduced by up to roughly half, a loss of about 4.5 to 6.1 millimetres, with around two thirds of that reduction occurring within the first three months after extraction.¹ This early, rapid phase is one reason dentists often recommend deciding on a replacement plan sooner rather than later, since a shrinking ridge can later complicate implant placement.

What happens to the surrounding teeth?
Teeth are held in position partly by contact with their neighbours and by the opposing tooth they bite against. When a tooth is missing, adjacent teeth can gradually tilt or drift into the empty space, and the tooth in the opposing arch may over erupt because it no longer meets resistance. A review examining public awareness of tooth loss summarised the established consequences: drifting, tilting, or over eruption of neighbouring teeth, which in turn can lead to uneven wear, food trapping, and a higher risk of gum disease and further tooth loss.² These shifts also tend to happen slowly, so patients often only notice the change once the bite already feels different.
Beyond the gap: bite, jaw joint, and chewing
As neighbouring teeth shift and the bite changes shape, the way the upper and lower jaw come together can also change. Some patients notice they favour one side when chewing, or that certain foods become harder to manage. Because chewing efficiency depends on having enough opposing tooth pairs in contact, losing several posterior teeth without replacement can reduce how effectively food is broken down.³ Uneven forces on the remaining teeth and jaw joint have also been associated with discomfort in some patients, although individual risk varies considerably and not everyone experiences noticeable symptoms.
Does every missing tooth need to be replaced?
The evidence here is more nuanced than it may first appear. Dental research on the "shortened dental arch" concept, that is, an arch with the front teeth and premolars intact but without the furthest back molars, has found that many patients can maintain adequate chewing function and comfort even without replacing every posterior tooth, provided the remaining teeth are stable and well distributed.⁴ This does not mean replacement is unnecessary. It means the decision should be individualised: it depends on which tooth is missing, how many teeth are missing, the stability of the bite, and the patient's own comfort and goals. A dentist can assess these factors directly, which is why a clinical opinion matters more than a general rule.

Depending on the clinical situation, options generally include a dental implant with a crown, a fixed bridge supported by neighbouring teeth, or a removable partial denture. This is a decision best made with a dentist after a clinical examination, rather than a generic recommendation.
